It is important that you find out as much information as possible about what the optometrist found during the eye test. You can ask your optometrist (optician) to fill in this form. It will help you and your supporters to understand your eye sight and the health of your eyes.
There are two ways of filling in this form:
- It can be printed off and written on
- It is a PDF that can be filled in and saved on a computer
